Click on Reservation Form below and email the form to email@example.com and pay by credit card (preferred) using the button below, or print the form and mail it with your check to the address provided. Note that mailed checks must arrive before the reservation deadline. An alternative is to set up the Scientists' Society of Southwest Florida in your bank's Bill Pay system. In the "memo" provision for the bank's bill pay, if you have added funds for dues or contributions, please indicate how your funds should be allocated, or email your allocation to firstname.lastname@example.org. We want to be sure to meet your wishes.
Please mail your check at least one week prior to the meeting to ensure we receive it prior to the cut-off date. Our venues require us to provide an accurate reservation count three days prior to the meeting. For late reservations or changes, you may contact Herb Schneider, Treasurer by phone or email. There is often some limited flexibility to change the number of reservations. Note that no payments will be accepted at the door.
The venues will bill us whether or not the persons making the reservation attended. Please understand that we must bill no-shows to cover these costs.
